While Intervention Central is designed for educators, I'm using the Academic Survival Checklist Maker as a parent to design a customized checklist for my son focusing on study skills and time management. I can download and share what I create with my son's teachers. Checklists can be made for Homework, Note-Taking, Organization, Study Skills, or Time Management. All the checklist items in the drop box can be edited. There are options for adding notes as well. This resource is easy to use and I love that my son can be an active participant in creating his checklist.
